Our
gastro-intestinal tract is made up of the esophagus, stomach, duodenum,
upper intestine, and the colon. Each component has its own special environment
in which it functions. For instance the stomach produces large quantities
of acid for digestion, but reflux of this acid into the esophagus causes
pain, and heartburn. The upper intestine requires a much more alkaline
environment than the stomach to function properly. When the proper conditions
within the GI tract are not properly maintained, problems can arise, such
as ulcers, or infections caused by harmful bacteria and yeast. Part
of what helps maintain the proper environment in the G.I. tract is the
presence of several kinds of helpful or friendly bacteria in the gut.
These bacteria not only aid in digestion of food, but also help provide
vitamin-K for blood clotting, they help maintain proper pH, aid in the
absorption of vitamins and minerals, and help inhibit the unwanted growth
of harmful gut pathogens such as Clostridia difficile . Within the
confines of the small intestine and colon our body plays host to several
pounds of helpful bacteria including Escerichia coli (E-coli or coliform
bacteria), Lactobacillus acidophilus, and Bididobacteria bifidum.
In fact; a healthy GI tract will produce feces which are about two thirds by weight bacteria. When we take antibiotics this delicate balance is upset because the antibiotics we take don’t just kill bacterial pathogens in the blood stream, but also kill the good bacteria in our gut. That is why the largest side effect of any antibiotic has always been, cramping, GI pain, and diarrhea. When prolonged diarrhea occurs, this sets up a favorable condition for what is called an opportunistic infection to occur. This is to say that there are harmful bacteri just waiting in the gut for the chance to proliferate, and when the conditions are right harmful organisms start to multiply in numbers large enough to upset the balance of good bacteria and cause problems. There are many such opportunistic bacteria and fungi that are in your gut even as you are reading this, many people are carriers without any ill effects because their G.I. tracts are healthy. One of the more common, and more serious pathogens is Clostridia difficile . Clostridia difficile is an anaerobic, spore forming bacteria, which can produce both a gas, and a necrotizing toxin. That’s a toxin that can dissolve human flesh! If Clostridia infections are not caught and treated in time they can cause rapid and irreversible damage. Clostridia difficile and even the more serious C. perfringens can cause severe damage to the intestines, and large sections of the intestines may have to be removed because of a complication known as gas gangrene. The Clostridia bacteria attach themselves to the intestinal wall and cause ulcerations, this leads to inflammation and inflammatory chemicals cause the blood vessels to become leaky, and they will pour fluids into the gut, and more worrisome is that the infection in the gut can get blood borne. The build up of pus and dead tissue in the gut creates a more anaerobic environment, which actually helps the bacteria proliferate more, and a vicious circle of infection and inflammation begins to devour the gut. This progressive ulcerative condition is known as psudomembraneous colitis and it can become life threatening. While this all sounds terrible the incidence in patients taking a good acidophilus/bifida supplement is extremely low. In fact the most common place to get a Clostridia difficile infection s in the hospital. (Infections that are routinely transmitted while in hospitals are known as nosencomial infections.) The nosencomial transmission of C. difficile is about ten times more common than patients becoming infected outside the hospital environment. Doctors and nurses who don’t wash their hands properly between patients, and transfer the fecal contamination directly to the patient, or to their food transmit it from one patient to another. So if you enter a hospital and become infected with Clostridia difficile , don’t let them blame it on your antibiotics, it ten times more likely that an unclean health care worker transmitted it! How does Clostridia take hold in the gut? The gut is made up of millions of small convolutions of the gut membrane known as villi. The nurturing environment of friendly gut bacteria usually surrounds these villi. Prolonged antibiotic use can decrease these friendly bacteria by as much as a third. (Rarely do antibiotics ever kill more than a third of the gut bacteria.) Once the friendly bacteria is depleted, the bowel membrane is left unprotected, and other bacteria start to proliferate. The bowel may try to react by flushing itself with fluids; this is done by the large intestine actually pulling fluid out of the capillary bed, which is in contact with the gut membrane. This causes diarrhea, which causes dehydration, and a more favorable environment for pathogens to rule. The prolonged use of antibiotics causing a depletion of good bacteria, can also cause yeast, fungus, and other species of bacteria to proliferate, including: Staphylococcus aureus, pseudomonas species, proteus species, and the yeast Candida albicans (and occasionally Candida galabrata) are the most common secondary infections of the gut. The best cure is prevention by prophylactic taking live, active acidophilus and bifida supplements. If diarrhea does occur while taking antibiotics, it is important to immediately stop taking antibiotics, and increase your intake of friendly bacteria. This is done by taking supplements such as freeze dried acidophilus/bifida capsules and or live active yogurt. Lactobacillus acidophilus - protects the upper intestine Bifidobacterium bifida - protects the colon It is the bifida bacteria which does the most to prevent diarrhea, because it protects the colon. It is the colon, which regulates the amount of water added to the stool which causes diarrhea. Products such as Pepto Bismol™, Lomotil™, and should only be used after the discontinuation of antibiotics, and attempts at bacterial supplementation fail. If this still does not control the diarrhea, or if you experience severe pain, bloating, distention, gas, or bloody stools, then it is time to see a doctor and have a stool sample cultured. If treatment for Clostridia is initiated Flagyl (metronidizole) is the drug of choice in Lyme Patients, because it is biologically active against the Lyme spirochete, as well as Clostridia difficile. =:::~-->> page one - two - three - four |
